Valley Community Members

     We would greatly appreciate it if you could kindly watch the short video here (also located on the Valley High School Facebook page) and then use the MySketch tool to select the top 5-6 competencies that you feel are important for a Valley graduate to possess. Vaccination Information-Children 12 years and older

Maine residents age 12 and older can now receive a COVID-19 vaccine

Dear Parent or Guardian,

MSAD 13 was asked by the Department of Education to share the following information with you. Please be aware that Maine residents age 12 and older can now receive a COVID-19 vaccine. The COVID-19 vaccines have been shown to be safe and remarkably effective at preventing severe disease and death from COVID-19.


Scheduling Your Appointment

Currently, only the Pfizer-BioNTech vaccine has been authorized by the FDA for use in youth ages 12-17. Please visit https://www.maine.gov/covid19/vaccines to learn which sites are vaccinating youth.

The vaccines are free, and many sites are providing vaccination with no appointment necessary. If you cannot access the website or need additional assistance to get an appointment, you can call the Maine COVID-19 Community Vaccination Line at 1-888-445-4111. Deaf or hard of hearing individuals can access the Community Vaccination Line by dialing 711 and requesting to be connected to 1-888-445-4111.


Permission from a Parent or Guardian

Permission from a parent or guardian is required for individuals under age 18 to receive the vaccine. Parents or guardians can give permission in either of two ways:

· By phone with a witness listening on the phone line, as arranged by the vaccination site; or

· By written consent on paper or electronically.

Each vaccination site has a consent form. Follow the instructions for submitting the form provided by the vaccination site. For more information please visit: https://www.maine.gov/covid19/vaccines/public-faq.

If possible, youth should bring a form of identification to their appointment. If no documentation is available, parents or guardians can attest to the youth’s eligibility to receive the vaccine.
