Vaccination Information-Children 12 years and older

Maine residents age 12 and older can now receive a COVID-19 vaccine

Dear Parent or Guardian,

MSAD 13 was asked by the Department of Education to share the following information with you. Please be aware that Maine residents age 12 and older can now receive a COVID-19 vaccine. The COVID-19 vaccines have been shown to be safe and remarkably effective at preventing severe disease and death from COVID-19.

Scheduling Your Appointment

Currently, only the Pfizer-BioNTech vaccine has been authorized by the FDA for use in youth ages 12-17. Please visit to learn which sites are vaccinating youth.

The vaccines are free, and many sites are providing vaccination with no appointment necessary. If you cannot access the website or need additional assistance to get an appointment, you can call the Maine COVID-19 Community Vaccination Line at 1-888-445-4111. Deaf or hard of hearing individuals can access the Community Vaccination Line by dialing 711 and requesting to be connected to 1-888-445-4111.

Permission from a Parent or Guardian

Permission from a parent or guardian is required for individuals under age 18 to receive the vaccine. Parents or guardians can give permission in either of two ways:

· By phone with a witness listening on the phone line, as arranged by the vaccination site; or

· By written consent on paper or electronically.

Each vaccination site has a consent form. Follow the instructions for submitting the form provided by the vaccination site. For more information please visit:

If possible, youth should bring a form of identification to their appointment. If no documentation is available, parents or guardians can attest to the youth’s eligibility to receive the vaccine.

Thank you for all you have done to keep your student and our school community safe during the pandemic.

The RSU 83/MSAD 13 utilizes assessment data to help inform our practices at the district, school, classroom and individual levels for our students.  Although we encourage students and parents to participate fully in our assessment programs, it is important to note that parents/guardians may opt their child out of state testing.  If you are interested in doing so, please reach out to your child's principal to make this request in writing.
